Our team lives locally and are deeply rooted in the community with many years of providing exceptional hospice care. Everyone member of our team has a servant’s heart drawing from personal and professional hospice experience. Our Vision is to seek to be our community’s first choice when choosing hospice care. We believe that hospice should be patient-driven and family-centered as no two people are the same. A team approach to end-of-life care is pivotal to ensure patients and their loved ones have all physical, emotional, and spiritual needs met. In remembrance of our own loved ones lost along the way, our team has built a foundation of love that is reflected in our care for others. What is hospice?
Hospice is a specialized service for people with a terminal illness. Hospice provides pain and symptom management rather than curative care. The focus is quality of life and comfort while addressing physical, emotional, and spiritual needs. We care for people wherever they call home.
Who is eligible for hospice?
Who is eligible? Hospice is for people who have a life-limiting illness with a life expectancy of 6 months or less if the disease follows its normal course. Hospice should be considered sooner rather than later to receive the maximum benefits that this service has to offer.  Care is provided from a team of care professionals Physicians, Nurses, Hospice Aides, Social Workers, Spiritual Counselors, Bereavement Coordinators, Volunteers, PT, OT, Speech Therapists.
